Diabetes Wellness Program

Total Plan Services (in alliance with 4G Biometrics) is pleased to announce a new benefit for our insured members and/or their dependents who are diabetics. We would like to partner with you and your physician to provide you with new tools to help you better manage your diabetes. As part of this new service our benefits plan will provide you with a personal monitoring system that will help you conveniently track and manage your blood sugars.

This new program is optional, but your diabetic supplies will be provided at no cost to you under this program. So how does it work?
